• Registration Checklist   ( Fillable Check list PDF - For your use only)

• Registration and Waiver Form  (Fillable PDF - Download, fill on your computer, print,  initial, sign and have witnessed by non-family member. Then scan and email to Club Registrar) 

• In order to make the Registrar's job as easy as possible, please scan the Registration and Waiver form as ONE continuous PDF document instead of separate pages so that there is only one attachment sent to Registrar. 

• If you don't have a scanner with the ability to scan multipage documents as a single file, you can install the free Microsoft Lens app on your phone and use it. Look for a button that says "Add pages" or an icon with a camera with a plus sign.
